[Martha's apparently decided to sleep on the Plane now, and she's brought a bit of her surroundings with her. She's presently curled up beneath a bush, her head resting on a large pack and her coat wrapped tightly around her. She's asleep, clutching something with her right hand, but she sleeps lightly these days - approaching her will most definitely wake her up. The patch of land she's brought looks...well, dead would be the best way to describe it. Or like a post-apocalyptic wasteland.
A closer look at Martha herself might reveal that she's a bit younger than most of the Plane is used to, and possibly a little thinner. Her face is pale and drawn, and she does not look like she's been sleeping well for...well, months, probably. Add in a bit of stress (or a lot of stress), and it's fairly easy to tell that she lives her life close to her breaking point.]

Martha, look at me. [takes a breath, speaking as calmly as possible] You need to calm down, just for a few minutes, all right? I'm not sure what's going on. Maybe you're confusing me with another Doctor, I don't know. Just, try and explain to me what's happening, so maybe I can help.

Actually, last time I looked there were four Doctors with this face, and two others who looked like me, but aren't.
Maybe you two just haven't crossed paths for people to know.

[takes a breath, and then tries to give her a reassuring smile]
While you're here, you should at least clean up a little. You look like you could use a good meal, too. There's a kitchen and some beds around here somewhere.
[he pushes himself up to a stand, and offers her a hand]

[pauses, unsure how much he can say, and then finally makes a decision]
Just - I don't know how our worlds are different. But, I'm a little ahead of you at the moment. Although, if I'm ahead of you now, I probably always will be, barring some unseen circumstances. [shakes his head and forces himself to focus] Either way, you're going to win. [tries on that reassuring smile again] Martha Jones, you save the world.

[nods] You...aren't the first person to tell me that. And it's reassuring, it really is. It just doesn't seem like it'll ever be possible sometimes. But it's those times when the world needs me the most, you know? Whether or not I succeed in the end, I can't stop trying.
