You do mean the Doctor, I hope. I'd have to have admitted to punching someone other than him. That doesn't mean I haven't, but the confusion would be awful.

He could have naturally developed this madness. He's gone through ridiculous amounts of loss this regeneration and he has hardly recovered from the Time War, so it's possible he's just snapped. Not likely, but possible.

We know that our dear experimenters are perfectly capable of manipulating the mind with chemicals and, perhaps, illusions. Thus, the second possibility is that they have caused it. In which case, it could wear out as all experiments seem to, or, alternately, I could strap him down - he'll probably flail about how we can't take him, too, he will resist us, blah blah - and I'll look him over for psionic or chemical unbalances. Usually our brains compensate, but I suppose not always.

Possibility three is that it's an external force from his own world, which would have the same dealing-with method as possibility two, I imagine, only without the 'wait it out' option that one has.
